Sponsored by the state of Indiana, eligible Indiana health plan members receive medical care like, doctor visits, prescription drugs, mental health care, dental care, hospitalizations, surgeries, and family planning at little or no cost to the health plan member.

Who Is Eligible For Indiana Health Plan?
Children’s Health Plan
Children, up to age 19, and certain young adults up to age 20 (living with a caretaker relative), who meet Indiana’s income guidelines may be eligible for the Hoosier Healthwise health insurance plan.
Pregnant Women
Pregnant women in Indiana who meet income and assets requirements may be eligible for full or partial insurance coverage under Indiana’s Hoosier Healthwise health plan.
Low Income Families
Families who meet Indiana’s income/asset guidelines and who have children under the age of 18 living with them or other caretaker relative may be eligible for Indiana’s Hoosier Healthwise.
